   We recommend completing the application on a desktop or laptop computer, not on a mobile phone.
2. Required:
   Click here to open PDF with all required documents
   (Photo Release, Medical Form, Code of Conduct acknowledgement and Cancellation Fee Acknowledgment and get the required student and parental signatures.)
3. Required:
   Forward the completed and signed required documents to:
